Henry & Stark County Health Departments Are Conducting Community Health Needs Assessment Surveys

The Henry and Stark County Health Departments announce they are kicking off their IPLAN (Illinois Project for Local Assessment of Needs) process.  The IPLAN is a community assessment and planning process conducted every five years by the Health Department.

RaeAnn Tucker, Director of Health Promotion states, “Step one in our IPLAN process is gathering data from Henry and Stark residents as to what they see as health problems affecting their community.  Therefore, we are inviting bi-county residents to participate by taking a survey of community health needs.”

Residents can take surveys online by simply going to the following links:

Henry County residents:   https://arcg.is/r5bje1

Stark County residents:    https://arcg.is/0eD5T3

Spanish versions are available at these links as well.

In addition, the Health Departments have hard copies of the survey to fill out if anyone would prefer that means.  Anyone interested in obtaining a copy of the survey, can call our offices at (309) 852-0197 Ext. 249 and we would be happy to send them a survey.

Tucker concludes, “We ask county residents to participate in this important health survey.  And, we encourage residents to encourage friends and family to participate as well.  Your opinions are important.  This will help us to understand the health needs of our community and identify future activities.  The survey will take approximately 10 minutes to complete and all responses are confidential.
